While it is too early to determine exact rates, our goal is to pass through to Collaborating Partners the administrative portion of payments after basic operational costs.
Collaborating Partners will have the option to finance the Lead FI’s payroll. Collaborating Partners that don't provide financing will receive a reduced rate commensurate with current loan rates.
In order to join as a Collaborating Partner, the Collaborating Partner must agree to the following:
1. Submit the “CDPAP Care” Coalition Collaborating Partner Info sheet, and attachment C.
2. Submit a minimum of 3 letters of recommendation from your existing consumers.
3. Abide by the compliance rules as set forth by the CDPAP Care Coalition.
Please note, that once a Lead FI is approved the DOH will need to approve all new Collaborating Partners. The time to join, is now!
